Finding the right lipedema doctor: Here's how

Which doctor can diagnose lipedema? How can I tell if the doctor I have chosen has the necessary expertise to treat this condition? And how can I find a doctor I can trust?

Many women affected by lipedema ask themselves these and similar questions and set out to find "their" lipedema doctor.

Which doctor diagnoses lipedema?

In Germany, lipedema is mainly diagnosed by phlebologists (vein specialists) and vascular physicians, as their experience in ultrasound examinations enables them to rule out other possible causes of leg pain. The most common of these are vascular diseases of the legs, particularly painful varicose veins.

In addition, these specialists have dealt with the condition "lipedema" in greater depth during their medical training and specialization.

However, dermatologists and plastic surgeons also deal with lipedema. The newly created and recently published guidelines for the treatment of lipedema in Germany are a joint effort by phlebologists, vascular specialists (vascular surgeons and angiologists), dermatologists, and plastic surgeons under the leadership of the German Society for Phlebology and Lymphology. An experienced lipedema doctor, whether a phlebologist or vascular specialist, will therefore have the necessary expertise and knowledge to correctly diagnose and treat the condition.

Is there such a thing as "the" lipedema doctor?

The answer is a clear no. For the reasons mentioned above, there is no such thing as "the" lipedema doctor. Many general practitioners have also taken up this issue, as lipedema has increasingly come into the public eye in recent years—due to social media and affected celebrities—and the general practitioner is a trusted person and therefore the first point of contact for many people. If he or she is not familiar with the clinical picture of "lipedema," he or she will refer you to an appropriate specialist.

What distinguishes a "lipedema doctor"?

A doctor who specializes in lipedema is familiar with the multifaceted symptoms of this condition and can identify them by asking the right questions. Not every disorder of fat distribution or every case of excess weight that cannot be influenced by exercise or dietary changes is necessarily lipedema. This assumption is a widespread misconception, particularly promoted by relevant forums on the Internet.

If lipedema is suspected, the lipedema specialist will initiate the appropriate examinations and, once a diagnosis has been made, provide you with comprehensive advice on the various treatment options available.

A good lipedema doctor will never pressure you into choosing a particular treatment (conservative or surgical), but will always give you enough time to make a well-considered decision. What works for one patient may not be the right approach for another. The primary goal of treatment is always to reduce pain. Of course, appearance is also important to most lipedema patients, but in my experience, it usually takes a back seat to pain management.

How can I tell if my "lipedema doctor" is the right one?

When choosing your "lipedema doctor," always trust your gut feeling. It usually won't let you down. Therapy can only be successful if you can trust that the doctor takes you and your symptoms completely seriously. The success of a treatment depends on the relationship of trust and mutual respect between both parties. A good doctor will explain the options for each treatment and their advantages and disadvantages openly and honestly. This is particularly important when planning a surgical treatment. Of course, this does not mean that the complications mentioned will actually occur, but a well-informed patient is essential for successful treatment—whether conservative or surgical.

Some patients also come to my lipedema consultation several times to discuss one point or another again after careful consideration. Sometimes this leads to a change in the patient's attitude toward a previously conservative approach, and the patient wants to embark on a new course of treatment together with me as their "lipedema doctor." My job then is to support them fully and, if they wish to undergo surgery, to perform a stage-appropriate operation to improve their symptoms and, ideally, eliminate them completely.

In Germany, phlebologists, surgeons, plastic surgeons, dermatologists, and lymphologists specialize in the diagnosis and treatment of lipedema. In principle, the diagnosis of "lipedema" can be made by any physician who is familiar with the condition. Conservative treatment can therefore also be initiated by a general practitioner, but surgical treatment should be performed by a physician who is familiar with liposuction and has surgical training.

Diagnosing lipedema can sometimes be difficult, as the symptoms can be nonspecific. However, lipedema always involves symptoms, most commonly pain in the subcutaneous fatty tissue or a feeling of heaviness in the extremities. Increased fatty tissue without symptoms is definitely not lipedema, even though this is often suggested on the internet and in relevant forums. Increased fatty tissue without symptoms is referred to as lipohypertrophy and, from a BMI of 30, as obesity. Dimpling of the skin (cellulite), the inability to lose weight despite exercise and dietary optimization, or the rapid appearance of bruises are not indicative of lipedema, although they can occur with lipedema. Unfortunately, social media is often unhelpful and tends to cause fear when it comes to self-diagnosing lipedema. Instead, I recommend reputable literature, ideally the S2k guideline "Lipedema" published in 2024 by the Association of Scientific Medical Societies (AWMF), which is also understandable for medical laypeople. I use these criteria to diagnose lipedema, and we would be happy to discuss these criteria together during an initial consultation.

Consulting another doctor for a second opinion is always a good idea, especially if you as a patient feel that your symptoms were not adequately clarified during the initial consultation. In addition, even colleagues with experience in lipedema can sometimes be unsure about the diagnosis. Before undergoing surgery for lipedema, it is always advisable to seek a second opinion, as the vast majority of patients do not usually have lipedema.

The physician's experience with the condition "lipedema" is the key factor in diagnosing lipedema. Some symptoms may be nonspecific, but an experienced physician can make a definitive diagnosis by asking the right questions and performing a physical examination. Contrary to many claims on the internet, ultrasound is not helpful in diagnosing lipedema. It only serves to rule out other conditions (e.g., varicose veins) as the cause of the symptoms.
