What happens during liposuction?
During liposuction, excess fat deposits under the skin are specifically removed. The treatment focuses on the subcutaneous fatty tissue, which is suctioned out through a small incision (approx. 0.5 cm) using a fine suction cannula. It is important to note that fat in the abdominal cavity cannot be treated, only the fat under the skin.
In my Düsseldorf practice, I use a modern and particularly gentle technique—water-assisted liposuction (WAL). A fine jet of water breaks down the fatty tissue into smaller particles without damaging important structures such as nerves, lymphatic vessels, or blood vessels. The fat is gently removed and the skin is often tightened afterwards with a laser or argon plasma to achieve optimal results.
Liposuction can be performed under local anesthesia, whereby the anesthetic is injected directly with the water jet. General anesthesia is also possible upon request. To promote quick and uncomplicated healing, it is recommended that larger procedures be divided into several sessions, with one to two areas being treated in each session.
What happens to the extracted fatty tissue?
The fat removed during liposuction can also be reused in other areas of the body if desired. This is known as fat transfer or "lipofilling." Popular areas for such transplants include the breasts or buttocks, but the face can also be gently sculpted using the patient's own fat cells. However, to ensure that the fat successfully grows in its new location, it is advisable not to transfer too much fat to each area of the body—for example, a maximum of 250 ml of fat per side and session for the breasts.
What is the recovery process like after liposuction?
Depending on the area of the body treated, you will need to wear special compression garments for around 6 weeks, both during the day and at night. Immediately after the procedure, you will usually be mobile and able to return to your normal daily routine after just a few days. You can shower the day after the operation. The small incisions are closed with special adhesive strips and usually heal completely within 2-3 weeks. In most cases, the scars are minimal and fade over time so that they are barely visible.

Frequently asked questions about liposuction
Since, for health reasons, too much fat should not be removed in one operation (the upper limit for outpatient procedures is approx. 3-4 liters of pure fat, with exceptions possible within narrow limits), liposuction is always performed in several areas. On the thighs, for example, this would be the front, inner thighs, and knee joints in one session, and the outer and back of the thighs in a second session. The lower legs are treated separately in a third session. The abdominal wall and arms are also separate areas. This reduces the strain on the body caused by the suction, as not too much fatty tissue is removed in one session.
Liposuction in Düsseldorf costs between €3,000 ("man boobs") and €5,400 (liposuction on the lower legs). In many cases, it makes sense to follow liposuction with laser or argon plasma ("Endolift") heat treatment of the skin to minimize the risk of sagging skin. For cosmetic procedures, statutory value-added tax of currently 19% is added to this price. Medically indicated liposuction due to lipedema is exempt from VAT.
Your BMI, i.e., the ratio of your body weight to your height squared, should not exceed 40. There is no fixed upper limit in terms of kilograms; it always depends on the individual case. However, there is an upper limit to the amount of fat that can be removed during outpatient liposuction. Ultimately, only a personal consultation can determine whether you are suitable for liposuction due to your potentially very high body weight or whether other measures would be more appropriate in advance. If you are very overweight, liposuction should be performed in a clinic under inpatient conditions.
The internet and social media often report suctioning of significantly more than 3-4 liters of fat, but this is not pure fatty tissue but fatty tissue plus suctioned local anesthetic fluid (so-called tumescent local anesthesia), which can sometimes amount to 10-14 liters with the water jet technique. This quickly adds up to an impressive 12 liters (pseudo) fat or more in the suction bag. This trend is concerning because it triggers a senseless and unhealthy competition among patients to see who has had the most fat removed. Please always remember that your fat tissue is an organ and that you are losing your body's own tissue during this procedure. For your own safety, the amount of fat removed must not be excessive. In outpatient settings, no more than 3-4 liters of fat should be removed in a single session. Exceptions are possible under certain conditions and within narrow limits. However, even larger amounts of fatty tissue should only be removed under inpatient conditions and not on an outpatient basis. Therefore, please do not compare your amount of fat with the "amount of fat removed" from other patients.
Liposuction is an invasive procedure like any other surgery. This means that the usual complications associated with any surgery can occur, although these are rare in liposuction—even when performed correctly—such as wound infections, thrombosis, allergies to the medications used, or circulatory reactions. Bruising and swelling of the treated area after liposuction due to wound fluid and lymph are completely normal and actually always occur. Therefore, manual lymphatic drainage by a physical therapist and wearing a compression garment for several weeks after liposuction is absolutely necessary. During a detailed consultation, I will explain the possible complications of liposuction and the appropriate measures to take in detail and answer all your questions at your leisure.
Since I only perform liposuction on an outpatient basis in my private practice, there is an upper limit to the amount of fat that we can remove in a single liposuction session. According to G-BA guidelines, a maximum of 8% of body weight should be removed in a single surgical session. Patients with serious pre-existing conditions are also not suitable for outpatient liposuction. In this case, I recommend treatment in a hospital under inpatient conditions. The vast majority of patients can be treated very well on an outpatient basis in my practice.
Depending on the technique chosen (water jet or vibration) and the amount of fat present, liposuction takes between 2 and 3 hours per region. A region refers to different areas of the body. For example, there are three regions on the leg: Region 1: front and inner thigh with knee region, Region 2: outer and back thigh, Region 3: lower leg circumference. The arms and abdominal wall/flanks are each considered a separate region. Vibration-assisted liposuction (PAL) is faster than water-jet-assisted liposuction (WAL). In our practice, we offer both methods (PAL and WAL) and use them on an individual basis.
Fat cells do not grow back after liposuction. However, the remaining fat cells—like any other fat cell—can grow again if you gain weight, causing you to gain weight again in the area that was treated, although this will not be as noticeable externally due to the reduced number of fat cells. However, if your energy balance is incorrect after liposuction, you will gain weight in other areas of your body. Your body cannot simply make the excess energy in the form of calories disappear into thin air, but must store it in the form of fat—just in a different place (abdomen, arms, face, etc.). That is why it is so important to maintain a balanced diet and get enough exercise even after liposuction.
As a rule, you will need 1-2 weeks to regain your previous activity level after liposuction. Most patients do not go to work for about 1 week, although the exact duration ultimately depends on your occupation. A sick note is only possible for medically indicated liposuction due to lipedema. Liposuction for cosmetic reasons does not result in a sick note and must therefore be performed during vacation time, for example. Light physical activity is usually possible again 1 week after the procedure. On the day of the operation, you will already be mobile enough to go home on your own, and light walks in the first few days after the operation are beneficial for a quick and good healing process.
The skin over the area where liposuction was performed takes about one year to completely retract and tighten. Nevertheless, sagging skin flaps may remain after liposuction, like a kind of "empty sack." To minimize this risk, we are happy to perform additional heat tightening of the skin with argon plasma or laser ("Endolift") at the end of the liposuction procedure in the same session. The applied heat contracts the collagen tissue under the skin and also promotes the formation of new collagen, which makes the skin look smoother and younger. The treatment is usually painless due to the local anesthesia already applied for the liposuction (tumescent local anesthesia) and takes an additional 20-30 minutes. It is not absolutely necessary, but is usually recommended immediately after liposuction. We would be happy to discuss with you during a consultation whether heat tightening is appropriate and desirable for you. The cost of additional heat tightening is approximately €1,200.
The extracted fatty tissue can either be discarded or, if you wish, transplanted to another part of the body (known as lipotransfer or lipofilling). The recipient areas on the breasts, buttocks, and face are very well suited for fat transplantation. However, fat transfer must be discussed in advance of the operation, as in this case the suctioned fat tissue is collected in a specially designed sterile container, then processed and subsequently transplanted. The transplanted fat tissue has a 70% chance of healing in its new location, although this cannot be guaranteed and depends on many factors. A transfer of 250 ml of fat per breast can achieve an increase in breast size of half a cup size. There are additional costs for fat transfer, which amount to approximately $1,000.
To perform liposuction, I recommend consulting a physician with training and experience in a surgical specialty. As a specialist in surgery and vascular surgery, I am very familiar with the anatomy of the area of the body to be treated and know the risks of organ or vascular injury, which I can avoid in advance. Ultimately, however, you should always listen to your gut feeling.
There are several differences between liposuction for lipedema and cosmetic liposuction. Firstly, there are differences in the surgical technique: in the case of lipedema, damage to the lymphatic vessels by the suction cannula should be avoided as much as possible. For this reason, in the case of lipedema, suction is performed parallel to the lymphatic vessels on the extremities in the longitudinal axis. In cosmetic liposuction, transverse suction directions are also possible, which can achieve an even better tightening effect. Both types of liposuction—for lipedema and aesthetic liposuction—naturally aim to achieve the best possible aesthetic result. However, in the case of lipedema, the focus is on reducing the painful fatty tissue, although the aim is, of course, to achieve the most aesthetically perfect result possible.
The next difference lies in the further postoperative course: in the case of lipedema, the urgently needed postoperative lymphatic drainage is covered by health insurance, which is not the case with cosmetic surgery. The patient can also be signed off work after lipedema liposuction. This is not possible with a procedure performed for cosmetic reasons, and the patient must take vacation time for the procedure and the initial recovery period. Last but not least, in the case of cosmetic surgery, you should take out follow-up cost insurance to be financially secure in the event of possible serious postoperative complications (e.g., hospital stays). This is not necessary in the case of lipedema, as this is the treatment of a disease (ICD 10) and therefore treatment of postoperative complications is also covered.
There are two different types of anesthesia used in liposuction: First, there is pure tumescent local anesthesia (TLA), in which the anesthetic is injected into the fatty tissue under the skin by the surgeon while the patient is awake. After waiting for the anesthetic to take effect, the fatty tissue is then suctioned out. No additional anesthesia is used in this procedure. The other option is liposuction under anesthesia, either in the form of light twilight sleep (analgesic sedation) or general anesthesia. These two latter anesthesia procedures are then performed by an anesthesiologist. A procedure under local anesthesia is always recommended, as this eliminates the very low risk associated with anesthesia and reduces the overall strain on the circulatory system to a necessary minimum.
