konservative als auch operative Lip\u00f6dem Behandlung zu erhalten.<\/p><p>Das Lip\u00f6dem ist eine &#8211; leider immer noch oft verkannte &#8211; Erkrankung mit einer stets seitengleichen Vermehrung des Unterhautfettgewebes an den Beinen oder Armen, verbunden mit Beschwerden wie Schmerzen oder einem Schweregef\u00fchl in der betroffenen Extremit\u00e4t.<\/p><p>Die Diagnose wird in der Regel anhand des Beschwerdebildes und dem k\u00f6rperlichen Untersuchungsbefund gestellt (z.B. ein positiver Kneiftest). Spezielle Laborparameter oder eine bildgebende Diagnostik (Ultraschall, MRT) existieren beim Lip\u00f6dem nicht und dienen vielmehr dem Ausschluss anderer Therapie des Lip\u00f6dems bilden die richtige Ern\u00e4hrung und die sog. &#8222;Komplexe Physikalische Entstauungstherapie&#8220; (KPE), bestehend aus:<\/p><p class=\"p2\">1. Kompressionstherapie (flachgestrickt),\u00a0<\/p><p class=\"p2\">2. Manueller Lymphdrainage (MLD),\u00a0<\/p><p class=\"p2\">3. Bewegungstherapie und\u00a0<\/p><p class=\"p2\">4. Rundgestrickte Kompression wird haupts\u00e4chlich in der Behandlung des Krampfaderleidens verwendet.\u00a0<\/span><\/h3><h3><span style=\"font-size: 16px;\">Sollte das Tragen der flachgestrickten Kompressionsw\u00e4sche tags\u00fcber keine Besserung Ihrer Beschwerden erbringen, so ist eine Fortf\u00fchrung der Kompression meistens nicht sinnvoll. Keinesfalls kann durch die Kompressionsw\u00e4sche das \u00fcbersch\u00fcssige Fettgewebe weggedr\u00fcckt werden.\u00a0<\/span><\/h3><h3><span style=\"font-size: 16px;\">Kompressionsw\u00e4sche gibt es heute in den verschiedensten Farben und auch mehrteilig, um flexibel in der Kombination zu sein. Manuelle Lymphdrainage (MLD)<\/h3><p>Die Manuelle Lymphdrainage (MLD) beim Lip\u00f6dem ist in Fachkreisen zunehmend umstritten. Warum ? Bisher konnte sowohl im Ultraschall als auch im Kernspintomogramm (MRT) keine freie Fl\u00fcssigkeit beim reinen Lip\u00f6dem im Weichteilgewebe der betroffenen Extremit\u00e4t nachgewiesen werden. Folglich erscheint ein Abtransportieren nicht vorhandener Fl\u00fcssigkeit durch eine Lymphdrainage auch nicht sinnvoll.\u00a0<\/p><p>Zudem kann auch durch die Lymphdrainage &#8211; analog zum Kompressionsmieder &#8211; kein Fettgewebe wegmassiert oder weggedr\u00fcckt werden. Allerdings gibt es viele Hinweise darauf, dass die manuelle Lymphdrainage mit ihren sanften und streichenden Bewegungen schmerzlindernd wirkt und damit doch ihren Platz in der Lip\u00f6dem Behandlung besitzt.\u00a0<\/p><p>Erg\u00e4nzt werden kann die MLD noch durch eine h\u00e4usliche sog. Intermittierende Pneumatische Kompression (IPK), bei der an Beinen oder Armen angelegte Manschetten maschinell mit Luft bef\u00fcllt werden. Zum einen unterst\u00fctzt Sie Bewegung in Ihrem psychischen Wohlbefinden und damit in Ihrer geistigen Haltung gegen\u00fcber dem Lip\u00f6dem, zum anderen f\u00f6rdert sie eine Stabilisierung Ihres K\u00f6rpergewichtes und kann Sie so vor einer Progredienz in ein h\u00f6heres Stadium &#8211; vor allem ins Stadium III &#8211; sch\u00fctzen.<\/p><p class=\"p1\">In diesem Zusammenhang einige Worte zum Sport beim Lip\u00f6dem: Prinzipiell ist jede Bewegung gut, aber \u00dcbungen zum Muskelaufbau sind immer einem reinen Ausdauertraining vorzuziehen, da mit zunehmender Muskelmasse Ihr t\u00e4glicher Energie-Grundumsatz steigt und Sie dadurch schon per se mehr Kalorien am Tag verbrennen. Sie m\u00fcssen daf\u00fcr auch kein Fitness-Studio aufsuchen. Es gibt wundersch\u00f6ne \u00dcbungen f\u00fcr zu Hause oder im Freien, um mit dem eigenen K\u00f6rpergewicht hocheffektiv zu trainieren und Muskulatur aufzubauen.\u00a0<\/p><p class=\"p1\">Erg\u00e4nzt werden kann dies selbstverst\u00e4ndlich durch Ausdauertraining wie Joggen o.\u00e4., aber bei dieser Trainingsform kann sich der K\u00f6rper schnell an die Belastung gew\u00f6hnen und verbrennt eventuell nur noch wenige Kalorien.<\/p><p class=\"p1\">Finden Sie Ihren Sport, der Ihnen am meisten Spa\u00df macht, denn nur dann werden Sie ihn auch langfristig durchf\u00fchren.<\/p><h3 class=\"p1\">4. Hautpflege<strong><br \/><\/strong><\/h3><p>Eine gute Hautpflege ist immer dann wichtig, wenn durch das vermehrte Fettgewebe Hautfalten entstehen, in denen durch Schwitzen Infektionen oder ein Pilzbefall auftreten k\u00f6nnen. Sie sollten darauf achten, dass das Hautmilieu sauer und nicht basisch ist, wie es leider durch viele Seifen erzeugt wird.\u00a0<\/p><p>Das vermehrte Fettgewebe kann auch zu einem Aneinanderreiben der Oberschenkel im Schritt f\u00fchren mit Folgen f\u00fcr die Haut, insbesondere im Sommer. Ich empfehle bei Hautproblemen die Konsultation eines Hautarztes passt.\u00a0<\/p><p class=\"p1\">Manche Patientinnen bevorzugen Intervall-Fasten, andere schw\u00f6ren auf Low-Carb oder f\u00fchren die h\u00e4ufig empfohlene ketogene Ern\u00e4hrung durch, bei der nur wenige Kohlenhydrate zu sich genommen werden und dadurch eine dauerhafte Umstellung des Stoffwechsels mit Abbau der eigenen Fettreserven erfolgt.\u00a0<\/p><p class=\"p1\">Ich empfehle zur weiteren Vertiefung Ihres Wissens zu den einzelnen Ern\u00e4hrungsformen die entsprechende Fachliteratur.<\/p><p>Wichtig: Denken Sie immer auch an die Getr\u00e4nke, die Sie zu sich nehmen, denn diese werden oft vergessen. Wir trinken unbedacht viele Kalorien und t\u00fcckischerweise tragen Kalorien in fl\u00fcssiger Form nicht zum S\u00e4ttigungsgef\u00fchl bei.<\/p><p>Leider liegt die L\u00f6sung auch nicht in Light- oder Zero-Getr\u00e4nken, denn auch sie k\u00f6nnen &#8211; obwohl nicht sehr kalorienhaltig &#8211; \u00fcber den erzeugten S\u00fc\u00df-Reiz im Gehirn zu einer vermehrten Insulin-Aussch\u00fcttung im Blut f\u00fchren. Bei der PAL zerlegen hochfrequente Vibrationsbewegungen der Kan\u00fcle das Fettgewebe schonend; bei der WAL \u00fcbernimmt ein feiner Wasserstrahl diese Aufgabe. Beide Methoden schonen dabei Lymphbahnen, Nerven und Blutgef\u00e4\u00dfe.<\/p><p class=\"p1\">Der Eingriff kann in \u00f6rtlicher Bet\u00e4ubung oder in einer Teil- bzw. Dabei wird immer nur das Fettgewebe unmittelbar unter der Haut entfernt, eine Fettabsaugung von z.B. im Bauchraum ist nicht m\u00f6glich &#8211; nur in der Bauchdecke.<\/p><p class=\"p1\">Pro operativer Sitzung sollte nicht zu viel Fett abgesaugt werden, da die Hautschnitte zwar sehr klein, die Wundfl\u00e4che unter der Haut aber gro\u00df ist und das Entfernen von Fettgewebe einen Organverlust f\u00fcr den K\u00f6rper bedeutet.\u00a0<\/p><p class=\"p1\">Ambulant werden dabei meistens bis zu 3 Liter reines Fett in einer Sitzung abgesaugt, in Einzelf\u00e4llen manchmal auch mehr. Dies h\u00e4ngt auch immer mit der Kreislaufsituation des Patienten w\u00e4hrend der Operation zusammen.\u00a0<\/p><p class=\"p1\">Unter station\u00e4ren Bedingungen kann mehr Fettgewebe abgesaugt Die abzusaugenden Areale werden angezeichnet und Sie legen danach OP-Kleidung, die Sie von uns erhalten, an.\u00a0<\/p><p class=\"p1\">Im OP nehmen Sie auf der OP-Liege Platz und es erfolgt eine ausgedehnte Hautdesinfektion und ein steriles Abdecken der zu behandelnden K\u00f6rperregionen.\u00a0<\/p><p class=\"p1\">Nach einer \u00f6rtlichen Bet\u00e4ubung werden dann mehrere kleine Hautschnitte von ca. 0,5 cm L\u00e4nge gesetzt und dar\u00fcber mit Hilfe einer Kan\u00fcle zun\u00e4chst die Tumeszenz-Lokal-An\u00e4sthesie im abzusaugenden Fettgewebe eingebracht. Nach eine kurzen Einwirkzeit mit Einweichen des Fettgewebes wird dieses dann seitengleich mit einer Kan\u00fcle von ca. 4 mm Durchmesser in Wasserstrahl-assistierter Technik (WAL) abgesaugt. W\u00e4hrend der Absaugung sind Sie \u2013 im Falle einer \u00f6rtlichen Bet\u00e4ubung &#8211; wach und k\u00f6nnen essen und trinken oder Musik h\u00f6ren.\u00a0<\/p><p class=\"p1\">Am Ende der Absaugung erfolgt dann &#8211; je nach Hautzustand \u2013 zus\u00e4tzlich eine Hautstraffung mit Argon Plasma oder Laser, um herabh\u00e4ngenden Hautlefzen an den abgesaugten Stellen vorzubeugen.\u00a0<\/p><p class=\"p1\">Danach wird die noch verbliebene Tumeszenz-Fl\u00fcssigkeit manuell aus dem Gewebe ausgestrichen, die Wunden werden mit Steristrips (Klammerpflaster) verschlossen und im Anschluss das Kompressionsmieder angezogen, welches Sie von uns erhalten. Die Wunden sind in der Regel nach 14 Tagen abgeheilt und die Klammerpflaster k\u00f6nnen dann entfernt werden. Das Kompressionsmieder sollten Sie 6 Wochen lang Tag und Nacht tragen.<\/p><p class=\"p1\">Wichtig f\u00fcr den Heilungsprozess ist in dieser Phase eine intensive postoperative Lymphdrainage durch einen Physiotherapeuten (Krankengymnasten) mit Beginn in den ersten Tagen nach dem Eingriff. Dadurch wird das Wundwasser und die sich ansammelnde Lymphe aus dem behandelten Bein oder Arm abtransportiert und die h\u00e4ufig auftretenden Verh\u00e4rtungen im Weichteilgewebe gel\u00f6st.\u00a0<\/p><p class=\"p1\">Die Lymphdrainage muss in manchen F\u00e4llen auch \u00fcber einen l\u00e4ngeren Zeitraum fortgef\u00fchrt werden und entsprechende Behandlungstermine in der Physio-Praxis sollten schon zur Operation feststehen.<\/p><p class=\"p1\">Mit leichtem Sport kann nach ca. 4-6 Wochen wieder begonnen werden.<\/p><p class=\"p1\">H\u00e4ufig ist nach 3 Wochen die behandelte Extremit\u00e4t soweit abgeschwollen, dass schon ein engeres Mieder angelegt werden kann. Aus diesem Grund erfolgt die erste geplante Nachuntersuchung zu diesem Zeitpunkt in unserer Praxis.<\/p><p>Das Endergebnis der Liposuktion sehen Sie fr\u00fchestens nach ca. 6 Monaten. Allgemein g\u00fcltige Erholungszeiten gibt es nicht. In aller Regel sind Sie in der ersten Woche nicht arbeitsf\u00e4hig und ben\u00f6tigen eine gewisse Erholungszeit. F\u00fcr diesen Zeitraum erhalten Sie von mir eine Krankschreibung, bei Bedarf auch dar\u00fcber hinaus. Meistens kann aber in der zweiten Woche der berufliche Alltag wieder aufgenommen werden. Schmerzen bestehen in der Regel in der ersten Woche und sind vergleichbar einem starken Muskelkater im Bereich der abgesaugten Regionen. Sie erhalten von mir ausreichend Schmerzmittel und meine Handynummer, unter der ich stets zu erreichen bin (7\/24). Nebenwirkungen gibt es beim konservativen Therapieweg in aller Regel nicht. Bei der Fettabsaugung handelt es sich um einen operativen Eingriff und damit bestehen hier naturgem\u00e4\u00df die klassischen OP-Risiken wie Thrombosegefahr, Infektionsgefahr, Weichteilschwellungen oder Nervensch\u00e4den. Im OP-Aufkl\u00e4rungsgespr\u00e4ch werden von mir alle Risiken angesprochen und erl\u00e4utert. In aller Regel treten aber keine ernsthaften Komplikationen auf, postoperatives Schmerzen sind in den ersten Tagen normal und k\u00f6nnen durch entsprechende Schmerzmedikation gut behandelt werden. Eine gr\u00f6\u00dfere Gewichtsabnahme im Vorfeld einer Operation ist nicht sinnvoll, da dies das Risiko herabh\u00e4ngender Hautlefzen nach dem Eingriff erh\u00f6hen kann. Wichtig ist auch, dass Sie sich auf das OP-Aufkl\u00e4rungsgespr\u00e4ch mit Ihrem Arzt vorbereiten, indem Sie sich z.B. Fragen im Vorfeld auf einem Zettel notieren, die Sie m\u00f6glicherweise sonst w\u00e4hrend des Gespr\u00e4chs in der Aufregung vergessen k\u00f6nnten. Stellen Sie wirklich jede Frage, die Ihnen wichtig ist. Ein verantwortungsvoller Arzt wird alle Fragen mit Ihnen durchsprechen und versuchen, keinen Punkt offen zu lassen. An den Armen ist eine Ultraschalluntersuchung auch bei Beschwerden nicht sinnvoll, da hier keine Krampfadern auftreten. Dann wird zusammen mit der Patientin der bestm\u00f6gliche Therapieweg herausgearbeitet. Dabei wird in der ersten Sitzung das vermehrte und schmerzhafte Fettgewebe an der Vorderseite und Innenseite der Oberschenkel sowie der Knieregion der Patientin abgesaugt, in einer zweiten Sitzung dann die Au\u00dfen- und R\u00fcckseite der Oberschenkel und letztlich in einer 3. Operation die Unterschenkel zirkul\u00e4r. Die Reihenfolge der Operationen und die Frage, ob \u00fcberhaupt alle drei Eingriffe notwendig sind, richtet sich nat\u00fcrlich nach dem individuellen Befund und dem Wunsch der Patientin. An den Armen erfolgt der Eingriff in aller Regel in einer Sitzung, ebenso wird nur eine operative Sitzung f\u00fcr die Bauchdecken und\/oder Flanken ben\u00f6tigt. Eine Kombination mehrerer K\u00f6rperregionen sollte unter ambulanten Bedingungen nicht erfolgen, da dann zu hohe Fettmengen abgesaugt werden m\u00fcssen mit unn\u00f6tig hoher und m\u00f6glicherweise auch gef\u00e4hrlicher Belastung des Kreislaufes (Fettverlust ist Organverlust).
